Abstract
The workshop is organised by the INFN Strong-2020 (http://www.strong-2020.eu) community, in the framework of the activities of the Nt-6 Network Lattice Hadrons. It is devoted to the study of the phases and phase transitions in particle physics, within and beyond the standard model, with emphasis on the lattice approach. We foresee invited talk from a non-lattice experts, possibly, but not limited to, belonging to the larger Strong-2020 community. Lattice talks will be preferably given by younger colleagues, and we would like to reserve time for discussions. The five days format allows eight sessions (discarding Monday morning and Friday afternoon). A typical session may comprise the above mentioned introductory talk(s), discussion, break and topical talks; Thursday morning includes free time, and an organisational and planning meeting.

GREEN PASS
A *Super Green Pass* is currently required to access most facilities and services in Italy, including the GGI. A "Super Green Pass" certifies full vaccination or recovery. If you have a valid “Green Pass” fulfilling these two conditions you do not need to take further actions. More information is available on the Ministry of Health website https://www.salute.gov.it/portale/nuovocoronavirus/dettaglioFaqNuovoCoronavirus.jsp?lingua=english&id=230#11 and (in italian) on https://www.governo.it/sites/governo.it/files/documenti/documenti/Notizie-allegati/tabella_attivita_consentite.pdf
Regional and local authorities in Italy are empowered to adjust these measures where required, which may occur at short notice. You are strongly advised to consult the websites for the regions in which you are travelling for the latest information, and to check with health authorities if you doubt the validity of your certifications.
SELF-CERTIFICATION
In addition to be in possession of a ”Super Green Pass ", those entering GGI must fill in each day a self-certification concerning their health status. Body temperature readings may also be taken.
ENFORCEMENT OF THE RULES
The staff on duty at the enter of the Institute will deny access to those non complying with these rules. The "Super Green Pass" verification may require proof of identity, no personal information will however be stored.
FURTHER HYGIENIC MEASURES
Participants must wear face masks at all times in all GGI spaces, correctly covering nose and mouth. Alcohol-based solutions are provided at various points.The number of people simultaneously allowed in the same room is restricted, and lunch breaks are organised in such a way that meals may be taken in turns.
